    Sampling Station Identification Form



When sampling at a new location on a lake or stream, please fill out a sample identification form and submit it to the appropriate monitoring program (volunteer lake assessment program (VLAP) or volunteer river assessment program (VRAP)). This information is vital to mapping the sampling data to the correct waterbody and location. Without this information, the sample data may be of little use to others.

When filling out the form, please write clearly and leave no column unanswered. Also, please attach a map or aerial photo which shows the location of the station. The map can be from an atlas, USGS quadrangle sheet, or any other type of media shows a nearby street intersection for reference.
